Not long ago, we started using a web-based collection tool called FormFox that allows collections to be performed at any participating facility for any specimen type.

In the past, the collection process has been driven by paper forms. Laboratories would often pre-print client-specific information onto the forms, requiring a collector to inventory a different kind of form for each client.
That resulted in shelves full of forms, which were not only hard to fill out but were also prone to errors. It also left employers and service providers in the dark through much of the collection process and test result status.

How FormFox Works
Using FormFox to place a testing/collection order is easy and requires just five steps:
- To order testing, go to FormFoxOrder.com and set up a free account;
- Choose a lab with which you have an account;
- Fill out the order information complete with details about the donor using the lab’s account number. (FormFox associates the employer with that account number.);
- Choose Beaumont Occupational as your collection facility and schedule a test date;
- Print or email the authorization to the employee.
When you enroll an employee, FormFox creates an electronic chain of custody in real-time. It shows you the testing facility and provides a pre-authorization form containing a barcode that your employee (donor) takes to the collection site to complete the test.
Scanning the barcode into FormFox using a tablet (iPad) will either show the scheduled test in the system or allow the facility to begin the collection process from the initial steps.
The web-based collection wizard walks the collector through each step of the process and validates the information used to complete the CCF.
Once the form is printed, the collector places the form labels onto the bottles, acquires the donor signature and provides the donor with the receipt portion of the form. The remainder of the form gets packaged with the sample bottle(s) and sent to the lab.
Upon arrival, the lab scans and encrypts the form, making the donor information safe yet accessible by those who may need it. In the meantime, anyone involved with the collection can keep track online of the status of the test. Employers can know immediately when employees show up for their screening and whether a positive result was reported.
